The Discoid Lateral Meniscus


Abstract
A clinical review was done on the discoid lateral meniscus, which was diagnosed and treated at our Department from 1978 to 1984. Clinical examination, orthography, and arthroscopy were the major three diagnostic tools.
The discoid lateral meniscus was found in 47 knees of 45 persons, occupying 17.9% of all the meniscal lesions in our series.
The age distribution showed that 15 were in the second decade and 24 in the third decade. These two decades occupied 83% of all the cases.
Past history disclosed no contributory mechanism of injury in 27 cases (47.5%), apparently indicating that the discoid meniscus could easily be injured with no obvious trauma.
The disorder manifested itself with vague discomfort or joint pain in 18. 3% and with positive McMurray test in 23.0%. Click was absent in a large number of cases, being present only in 19.6%.
In compliance with the mechanism involved, the tear could be classified into five types. Most frequent was the longitudinal tear, which was 21 cases (44.7%) and six of which was typical bucket-handle type.
Excision of the damaged discoid meniscus was done with the prospect of preventing secondary degeneration of the joint cartilage and in this respect partial meniscectomy was preferable to total excision.
